Clinics
Our Practice provides a wide range of care
Our practice nurses are dedicated and patient centred in their approach, they are very much appreciated by our service uses. They provide special hypertension clinics, diabetes clinics, asthma clinics, contraceptive clinics, wound clinics, child immunisation and routine and travels vaccines etc.
Our GPs partners have years of experience, they offer medical advice and treatment on a wide range of condition and advice on how to prevent illness and maintain fitness. They are proactive, empathetic and always willing to go above and beyond to help the most disadvantaged, vulnerable and those at high risk.
We focus on the broader factors which influence patient’s health and well-being by adapting a holistic approach through managing both the physical and the mental health aspect.
We have Child/Adult multidisciplinary team meeting once monthly where the healthcare professionals involved in the patient’s care meet to discuss their care in a more holistic approach. This involves, the community matron, the health visitor, the GP team, the social worker and the community physiotherapist.
Blood Test Phlebotomy Service
Every Thursday between 8am and 12:50pm by appointment and for registered patients only.
Child Health
The post-natal clinics and child surveillance clinics are delivered weekly in conjunction with the nurse immunisations clinics to make it more accessible for families with new born babies.
Dietitian
Our dietitian works remotely only via telephone consultation by promoting health, managing diet-related conditions, offering dietary advice and personalised plan, weight management and preventative care in obesity and malnutrition and much more.
Mental Health
Our Mental Health Nurse provide a specialised support for patients dealing with mental health challenges including anxiety and depression: Assessment and diagnosis, treatment planning, medication management, ongoing emotional support, crisis intervention, Health education and advocacy. Based at the practice this reduces the time patient must wait to be seen by a mental health specialist service.
Physiotherapy
Our Physiotherapist is very skilled in her craft, she assists patients in recovering and maintaining their mobility, function and overall well-being. Patients are offered a personalised plans which includes exercises, manual therapy and other techniques to improve mobility and reduce pain. She assists patients who are recovering from surgeries, strokes or injuries to regain their independence and functionality. She offers advice on injury prevention, posture and lifestyles changes to enhance physical health. She is in high demand and slots are immediately filled, we therefore ask for your patience.
Social Prescribing
Our Social Prescriber works closely with the GPs and offers a range of services aimed at improving people’s overall well-being by addressing non-medical needs such as connecting patient to community-based activities and support systems, that can help with practical, social and emotional challenges particularly for those dealing with loneliness and isolation by signposting them to befriending services, support groups or other local community activities.
Travel Vaccines
We offer the following vaccines: Hepatitis A, Typhoid, (Tetanus, Diptheria, Polio) and Men ACWY.
