Registration Times

Covid-19 Pandemic –Please note that we are registering patient but the process is different, all you have to do is download and complete the *GMS1 Registration Form and *Health Questionnaire and email the forms to us along with proof of address and ID at [email protected]

Monday to Friday 12.30 pm to 13.00pm,  Except Thursday 12.00pm to 12.30pm

Monday to Friday 17.00pm to 18.00pm.

We register patients within E11 postcode only.

Your Data Matters

 

There is a waiting list for registrations up to 6 weeks at present. You will need to come into the Surgery with proof of address for over 18’s. eg.

  • bank statement
  • utility bill (not older than 3 months)
  • council tax bill covering this year
  • original tenancy agreement signed and dated

(you only need to show 1 of the above)

Please note driving licence and mobile phone bills are not acceptable.

To process your registration there is a minimum of two weeks within that period we cannot book you an appointment except in  genuine emergency.

As a general rule, all patients are required to show a valid passport as proof of ID.

Under 18’s will need a birth certificate and cannot be registered without a Guardian registered or being registered. (IF THE GUARDIAN IS NOT A PARENT WE WILL NEED TO SEE PROOF OF GUARDIANSHIP).

Also under 7’s will need proof of Immunisations status.

ALL UNDER 18’S AND PATIENTS WITH CHRONIC CONDITIONS WILL NEED TO ATTEND NEW PATIENT HEALTH CHECKS WITH THE NURSE. UNLESS NEW BABY BORN WHILST GUARDIAN IS A PATIENT AT THE SURGERY.

Incomplete registration forms will be rejected, please ensure that GMS1 and Health Questionnaire forms are fully completed and Proof of address and ID  are  attached.
